Open reduction / Internal fixation

개방 정복 / 내부 고정 =개방정복고정술

 

 

 

 

 

 

 

 

1. Indication(적응증) 

ZMC fx. : zygomatico maxillary complex fracture, 협곡 상악골 복합체 골절

 

 

 

2. 수술 방법

-마취 방법 : general, 전신마취

-수술 체위 : supine position, 앙와위

-수술 시간 : 약 2시간 이상, 케이스별 상이함

 

 

 

3. 수술 약품

-dental lidocaine(2%) *2 -> SC(subcutaneous,피하) inject

-1% lidocaine + epinephrine 0.2cc mix -> SC inject

-N/S 20cc*1 + epinephrine 0.2cc mix -> 출혈 부위 epi gauze packing용으로 사용

 

 

 

4.수술과정

OR (Open reduction, 개방 정복)

<입>

-small richardson*2 + yankaur suction tip assist한테 줌

-50cc syr.에 betadine irrigation *2 -> 50cc syr. N/S irrigation *2

-inject : dental lidocaine

-long pin + bovie 사용해서 입 염

-PRN) epi packing gauze 찾으면 4*4G에 뭍혀서 줌

 

<코>

-Nasal speculum + long pin + betadine cottonoid*2 로 코 안 닦음

-1% lidocaine + epinephrine 0.2cc + spinal needle에 연결해서 inject

-epi cottonoid packing

 

<눈>

-eye protector 착용 : oint 뭍히고 뾱뾱이 연결해서 줌

-prolene 6/0 tagging*7ea + fine T + mosquito str(3ea)

-dental lidocaine inject

-canthotomy(외안각절개술/눈구석절개술) : marking pen 얇은 부분으로 줌, mess#15, tenatomy sci

-ragnell, skin hook 2-prong, senn(prn), malleable(중), orbital ret 준비

-pf. 왼손에 orbital / 오른손 bovie dissection

-septal, ele(소)로 fx. 부위 bone 빼냄 * 이때 나온 bone 보관할지 확인(W/G에 보관)

-epi G packing + long fcp(M) : Bleeding control

 

<입>

-s-tip 10Fr. bovie power change

-richardson, Rt. angle(maxillary extra)*2, light ret

-ele(중,대) -> Fx. 부분 reduction 시켜줌

 

Open reduction 후에 PRN으로 portable 찍음 (+중방 필요)

 

Internal fixation (내부 고정)

*drill setting 할 것

 

<plate 준비>

-plate 잡을 때 : padson S or plate fcp 사용

-plate 모양 확인 -> prn)cutter, bender이용해서 모양 잡음

-plate, screw종류/개수 잘 적어놓을 것 ->자르고 안쓴 경우 처방만 넣고 기록지에 작성 안함

 

*PS)matrix 신형 set

-instrument : screw handle*2, cutter, bender*2, plate fcp ->사각 tray에 넣어놈

-plate : 0.4(높이)sliver/ 0.5 : blue / 0.7 : pink / 0.8 : gold

-screw : 1.5(4,6,8,10,12mm) / 1.85(4,6,8mm)

-drill bit : 1.1mm(빨강) -> 1.5 screw / 1.5mm(파랑) -> 1.85 screw

(plate pink, blue, silver -> 1.5 screw & 1.1빨강(4/6/8mm)drill bit / gold -> 1.85파랑(4/6/8mm)drill bit 사용)

*1.0 / 1.3/ 1.5/ 2.0 Plate  + PS)구형 Inst.(1.5/1.5 driver 있음)

 

*inion : 녹는 plate, screw로 주로 젊은 환자들 사용, 개봉시 소독 불가 신중히 풀 것!

-inion set, warmer 준비

-inion set : handle*2, driver*1, 1.5 drill bit/tapper, 2.0 drill bit/tapper

-warmer IR용(Inion) syr 고무줄로 표시(screw박으면서 틈틈이 wram IR함)

-Plate->drill->tapper->driver+screw->drill+tapper 닦기->drill 준비

-ZMC Fx시 : orbital, forehead(frontal), maxillary plate 밖음

  Frontal : 4H extened/Lot number. SCR1225

  Orbital : 6H 1.5*6mm/Lot number. SCR1223

  Maxillar : mesh type(7*7H) / 1.5*4mm(5ea짜리+2ea)

 

*Lefort 2,3 : maxilla까지 깨진 것 눈~입

-maxilla extra set

-IMF SCREW 준비(Roll wire, ent S/L)

 

*Mandible fx.

-maxilla extra set : mandible clamp, chin retractor

-single : wire twister(holder2, twister, cutter), maxilla extra(5ea or 3ea), ele septal, IMF screw, 구형 drill

-실 : vicryl3/0, vicryl4/0

-implant : 2.0plate & screw, 구형 inst, wire(200/300)

(implant routine 2.0 screw, drill bit 1.5(파랑), DCP 4H,6H Plate 사용, IMF screw 8mm는 drill 안쓰고 driver로만 고정)

Roll wire 동그랗게 모양 만듬 -> ent)s/l 얇게 2~3개 자름 -> mosquito

(wire NH = Mannual / wire twister=auto)

-oral IR로 시작

 

<IR & Suture>

입 : vicryl3/0(hegar NH, long T, Mayo) ->vicryl4/0

Prn)hemostop

눈 : PDS5/0 -> Vicryl5/0->mersilk7/0->vicryl6/0->

Dressing : mepilex, oint, sci, 뾱뾱이로 eye shield 제거 -> anti mix안된 물로 IR
