외측직근 사시수술

Indication : IXC(Intermittent exotropia), 간헐성 외사시

1. 수술 준비
-양안 PKG, MT, ,basic set, 가운(1), prep bowl, F/J, D)4*4G, D)2*2G
-10cc syr., surgidrape(6661)
-짧은 면봉, bipolar, 무영등 Handle, strabimus set
-suture : merslik6/0, vicryl 7/0
-premedi : saline, 멸균증류수, 포러스 연고


2. Setting & 상차림
-bipolar power : 3
-painting : betadine (2알)
-무영등 사용
-의자 가운으로 싸기
*Micro 사용 안함, PACS녹화 안함
-basic set : t/c, mosquito, crile, metzem, cut sci
-micro basic set : castro 0.12, tenatomy sci, muscle hook, needle holder(2), caplier
-strabimus set : speculum, locking castro fcp(2), 2-hand, muscle hook(ball tip), 일자 muscle hook
-H/T cornea 모양, 크기로 잘라서 준비해두기(양안일 경우 2개)
-surgidrape 1/4크기로 잘라서 준비해두기(양안일 경우 2개)

3. 수술과정
-surgidrape 1/4 자른 것 붙임(양안일 경우 보통 왼쪽 먼저 시행)
-metzem으로 자름
-speculum
-mersilk6/0 suture -> eye perotection(핸드타원 cornea모양으로 잘라뒀던걸 cornea에 붙여 보호함) ->moqsuito tagging
-castro0.12 + tenatomy로 conjunctiva dissection
-muscle hook(ball tip)->muscle hook(sharp)->assist tenatomy -> muscle hook sharp에서 ball tip으로 바꿔주고 muscle hook ball tip 2개로 lateral rectus검
-bipolar로 bleeding contro하고 tenatomy로 정리
-vicryl7/0 double arm으로 lateral rectus suture
-tenatomy로 cut
-0.5 locking castro fcp 2개로 잡고, assist 2-hand hook으로 검
-caplier로 길이 잼
-vicryl 7/0 suture
-dressing : 포러스 연고, 2*2G
